Purpose of this assignment
This assignment will
enable you to reflect on your professional practice through a portfolio of
observations and written work, which will be completed over the duration of
the course. In addition you will need to complete a minimum of 400 hours work
experience.

Unit 4 – Personal  and professional development in health and social care
should refer to the assessment criteria and also the relevant unit content,
when preparing your evidence for assessment.
Scenario
Working
as a new employee within Health and Social Care you are going to need to
complete some further research into care values and relate these to your own.
This will help you set targets for future work and development.
Task 1 (LO1 1.1 1.2 1.3 and M2)
You
are going to create an autobiographical
piece of writing. This will comprise the start of your portfolio. You
should begin with an introduction into the principles of health and social
care support, before outlining your own personal values and principles and
then comparing the two.
You
will then use the information to assess your own role in supporting the
service users you currently work with. Before lastly discussing new
developments and changes to your values can impact on working within the
sector.
To
achieve M2 you
must select appropriate resource materials when researching the values of
care with the use of the internet, journals, publications and text. The
bibliography must be presented in an appropriate format and include at least
three sources.

Task 2 (LO3 3.1
3.2 3.3 and M1)
You are going to give a presentation using PowerPoint to new employees within your work
setting, about the principles of professional engagement within the workforce
and with service users. The presentation will be titled ‘Professional Engagement’
and run for 5 – 10 minutes.
Start with an introduction into professional
relationships and describe your role within the company. You will then explain
the nature of three different professional relationships within your setting,
including external agencies. Think about what problems that could arise
within these relationships and give examples of how to overcome them.
Lastly you will evaluate personal effectiveness
in promoting and supporting the rights of the individual within your
workplace.
To
achieve M1 you
must demonstrate that you have looked at various problems, which may occur
within the relationships and made effective judgements on how to resolve the
issues.

This task will be
developed across year in the form of self assessments, action plans and
reflective writing. The evidence will need to include:

To
achieve M3 the
action plans should be created using a structured sequence, which best suits
your style of learning. You will need to use this same template over two
years and include all of the information above.
To
achieve D3 your
action plans and relfections will need to include a thorough self-evaluation on
your personal and practical skills including any areas you could improve in
the future.
To
achieve D2
you should show an effective approach to
independent study and planning through your action plan. You also need to meet
the deadline for submitting all the tasks and achieve the unit assessment
criteria.

As you develop through
the course and within your placements it is important to show that you can
put the theory into practice. The main evidence for this task will be witness
statements and personal reflections. You will need to include four examples
of demonstrating effective teamwork, these will need to include some of the
practice you have been taught:

For each example you
will need to submit:

A reflection using all the criteria for learning outcome 4 of this unit
as sub-headings.

achieve D1 you
must show self-criticism when evaluating your role within the team, drawing
on conclusions to suggest further room for development (remember these can be
added to your action plan).
